SMS Alert Notification Consent

How medical professionals opt in to receive patient alert notifications through the Active SOS platform.

About This Messaging Program

Active SOS is a closed, B2B physician alert notification platform. It is not a consumer marketing service. The platform enables patients to send health alert notifications through the Active SOS mobile app, which are then forwarded as SMS messages to the registered physician or designated on-call staff at the patient's medical practice.

All SMS message recipients are:

  • Licensed physicians or authorized medical staff
  • Employees of a medical practice that has registered with Active SOS
  • Individuals who have explicitly provided their mobile number during practice onboarding
  • Individuals who have actively opted in using an unchecked consent checkbox (described below)

No consumer marketing, promotional, or solicitation messages are sent through this program. All messages are operational patient alert notifications.

How Opt-In Consent Is Collected

Consent is collected during practice onboarding in the Active SOS Provider Portal. Each physician or staff member who will receive patient alert SMS messages must individually complete this step. The consent checkbox is unchecked by default and is presented separately from the Terms of Service and account creation requirements.

Important: The SMS consent checkbox is always unchecked by default. A physician or staff member can complete practice onboarding and use the Active SOS platform without consenting to SMS notifications. SMS opt-in is entirely voluntary and separate from the required Terms of Service acceptance.

The following are representative examples of messages sent through this program. All messages identify Active SOS as the sender and include opt-out instructions.

Sample 1 — Urgent SOS Alert:
Active SOS URGENT: Patient [Patient Name] has triggered an SOS at [Practice Name]. Message: "I need help." Forwarded to your practice. Reply STOP to opt out.
Sample 2 — General Patient Alert:
Active SOS Alert: Patient [Patient Name] (+[Phone]) sent a health alert. Message: "chest pain." Review required in the Active SOS triage portal at activesos-portal.netlify.app. Reply STOP to opt out.
Sample 3 — Informational/Safe Alert:
Active SOS: Patient [Patient Name] sent a Safe Alert at [Practice Name]: "took medication." Queued for review. Reply STOP to opt out.
Sample 4 — Portal Review Notification:

Privacy & Data Handling

Active SOS does not sell, rent, share, or transfer mobile phone numbers or SMS opt-in data to any third party for marketing or promotional purposes. Mobile numbers collected during onboarding are used solely to deliver operational patient alert notifications.
