The Active SOS Health Monitoring SMS Program is a Remote Patient Monitoring (RPM) service that allows your healthcare provider to check in on your health through automated text messages. You will receive daily health check-in prompts and may receive additional messages based on your care plan and clinical needs.
Messages are sent on behalf of your enrolling healthcare provider or practice. Active SOS powers the technology behind the program.
This is a healthcare service — you will only receive messages related to your clinical monitoring. You will never receive advertising or unrelated content.
Your healthcare provider may ask you to sign a paper consent form during a hospital discharge or office visit. The form includes all disclosures and opt-out instructions.
Text the keyword below to the Active SOS program number provided by your healthcare provider. You will receive an immediate confirmation message.
