B2B Healthcare Provider Program

SMS Alert Notification Consent

How licensed healthcare providers and authorized medical staff voluntarily opt in to receive patient alert notifications through the Active SOS platform.

About This Messaging Program

Active SOS operates a closed, business-to-business (B2B) patient alert notification platform exclusively for registered medical practices. This is not a consumer marketing service. No promotional, advertising, or solicitation messages are sent through this program.

When a patient sends a health alert through the Active SOS mobile app, that alert can be forwarded as an SMS notification to the registered physician or designated on-call medical staff at the patient's practice — but only if that provider has individually and voluntarily opted in to receive SMS notifications.

SMS notifications are entirely optional. Providers can create an account, complete onboarding, log in to the portal, manage patients, review alerts, and use all platform features without ever opting in to SMS. Declining SMS consent does not restrict access to any feature.

All SMS recipients are:

How Consent Is Collected

Consent is collected in the Active SOS Provider Portal on the My Profile page, which is accessible at any time after account creation. Each provider who wishes to receive patient alert SMS messages individually checks an optional consent checkbox on their profile.

The SMS consent checkbox is unchecked by default and is entirely separate from the Terms of Service acceptance. Providers may accept the Terms of Service and create their account without selecting the SMS checkbox. They may opt in later — or never — without affecting their access to the platform in any way.

Account creation does not require SMS consent. A provider cannot receive SMS messages until they have individually and voluntarily checked the consent box on their Profile page. No SMS messages are sent before that action is taken.

The following is a representative example of the consent form as it appears in the Active SOS Provider Portal on the My Profile page:
