Patient SMS Consent

How patients enrolled in an Active SOS Remote Patient Monitoring program opt in to receive automated check-in texts and alert-response confirmations from their care team.

About This Messaging Program

Active SOS operates an SMS program on behalf of enrolled healthcare practices to support Remote Patient Monitoring (RPM). This program sends two kinds of messages to patients: (1) scheduled check-in texts based on the patient's individual care plan, and (2) confirmation/response messages sent back to a patient after they submit a health alert through the Active SOS app. This is not a marketing, advertising, or solicitation program.

How Patients Enroll — Two Opt-In Paths

Patients enroll in the Active SOS RPM check-in program in one of two ways, depending on the care setting:

Path 1 — Paper Consent at Discharge

Completed in person, typically at hospital or practice discharge. The patient (or their authorized representative) signs a form authorizing SMS check-ins and provides a mobile number.

Path 2 — SMS Keyword START

The patient texts START to the practice's Active SOS RPM number and receives an automated confirmation before any check-ins begin.

The following is a representative version of the paper consent form used at discharge. It collects the patient's name, mobile number, and affirmative written consent before enrollment.

Path 2: SMS Keyword START

A patient who has not completed paper consent may enroll by texting the keyword START to the practice's Active SOS RPM number. No check-in messages are sent until this step is completed.

This consent is specific to the RPM SMS program described here. It is separate from, and does not affect, the patient's use of the Active SOS mobile app for one-tap alerts.

Message Frequency & Types

Message frequency is set by the patient's individual care plan and varies by practice and clinical need. All messages sent through this program are operational RPM messages only:

No marketing, promotional, or third-party messages are ever sent through this program.

Sample 1 — Scheduled Check-In
Active SOS: How are you feeling today? Rate 1-10. Reply STOP to opt out.
Sample 2 — Medication Check-In
Active SOS: Have you taken your medications today? Reply STOP to opt out.
Sample 3 — Alert Response Confirmation
Active SOS: Your alert was received and forwarded to your care team. If this is an emergency, call 911. Reply STOP to opt out.

Privacy & Data Handling

Active SOS does not sell, rent, share, or transfer mobile phone numbers or SMS consent data to any third party for marketing or promotional purposes.

Mobile numbers collected for this program are used solely to deliver operational RPM check-in and alert-response messages. Data handling complies with HIPAA, TCPA, and CTIA Messaging Principles.
